Grass Valley, Calif. March 23, 2017 – Sierra Nevada Memorial Hospital Foundation is pleased to announce that the Remember Me: The Face Behind Dementia Campaign has surpassed its goal of raising $200,000 by 2017 to expand the services of the Alzheimer’s Outreach Program (AOP) at Dignity Health Sierra Nevada Memorial Hospital (SNMH).

Managed through the hospital’s Home Care Department, AOP provides crisis navigation and support to about 10,000 local caregivers caring for a friend or loved one with Alzheimer’s or dementia.

The over $200,000 raised during 2016 will expand one-on-one support, increase healthcare navigation, assist with access to respite care, increase education for caregivers, provide new training for first responders and medical professionals and help further community outreach and education.

Although SNMH Foundation has met this very important goal, Executive Director Kimberly Parker says there are plans to continue to expand the program. SNMH Foundation board members, committee members and staff will continue to campaign throughout 2017.
“By raising awareness and securing the funding for this program, the Remember Me: The Face Behind Dementia campaign has called attention to one of our community’s priority health needs,” said Parker. “Alzheimer’s isn’t a disease that only affects health. It also impacts finance, lifestyle, family and much, much more.”

Thanks to the community’s profound generosity, SNMH will continue to provide much needed assistance to an estimated 3,000 western Nevada County residents living with Alzheimer’s or dementia.
