GRASS VALLEY, Calif. (September 3, 2026) — Hospitality House announced today that, as of September 1, 2026, it began to offer Enhanced Care Management (ECM) to clients with complex needs, in partnership with Chapa-De Indian Health, a nonprofit health center. ECM is a statewide Medi-Cal benefit, funded through California’s CalAIM initiative, available to eligible members at no cost, that provides a single, dedicated care manager to coordinate primary care, specialty care, dental care, mental health treatment, and substance use disorder services for each person enrolled.
A neighbor struggling with homelessness is rarely facing a single problem. Untreated illness, unmanaged mental health or substance use disorder, and missed referrals compound one another — addressing them takes time most systems are not funded to provide. Enhanced Care Management funds that time, treating each need in relation to the others.
“Our clients’ needs are rarely isolated; they’re layered, and they affect each other,” said Sydnie Day, Care Services Manager at Hospitality House. “Enhanced Care Management funds the time to coordinate care across all of it, instead of only responding to the most immediate need.”
Chapa-De Indian Health providers will collaborate on case plans alongside Hospitality House’s care management team, aligning clinical treatment with the housing and stability services Hospitality House already provides.

Through this partnership with Hospitality House, we can help even more community members access the coordinated support they need to improve their health and well-being.”
